Toestemming en Behandelovereenkomst
Voor het uitvoeren van therapeutische behandelingen bij Pure Beauty and Health is toestemming van de cliënt vereist.
Door het plannen en ondergaan van een behandeling verklaart de cliënt dat hij/zij:

Vrijwillig toestemming geeft voor de uitvoering van de behandeling door de therapeut van Pure Beauty and Health.

Begrijpt dat de behandeling onderdeel uitmaakt van complementaire zorg en geen vervanging is van reguliere medische zorg.

Op de hoogte is dat de behandeling valt onder de beroepsorganisaties CAT, NWP, SKIN Register en ANBOS, en dat de therapeut handelt volgens hun beroepscodes en kwaliteitsrichtlijnen.

Volledige en juiste medische informatie heeft verstrekt, inclusief gebruik van medicatie, allergieën en relevante gezondheidsklachten.

Begrijpt dat de therapeut geen medische diagnose stelt, maar werkt binnen het kader van complementaire en natuurgerichte zorg.

Op de hoogte is dat er een cliëntendossier wordt bijgehouden waarin de gegevens van de behandeling worden vastgelegd, conform de AVG en de Wkkgz.

Kennis heeft genomen van de Privacyverklaring en akkoord gaat met de verwerking van persoonsgegevens zoals daarin beschreven.

Begrijpt dat het cliëntendossier 20 jaar wordt bewaard en de financiële administratie 7 jaar, conform de wettelijke verplichtingen.

Weet dat hij/zij te allen tijde recht heeft op inzage, correctie of verwijdering van persoonsgegevens, tenzij wettelijke bewaarplichten dit verhinderen.

Op de hoogte is van de klachtenprocedure via de GAT Geschillencommissie of de beroepsorganisatie waarbij de therapeut is aangesloten.
De cliënt verklaart kennis te hebben genomen van bovenstaande punten en gaat akkoord met de behandeling en verwerking van persoonsgegevens conform de geldende wetgeving.
